Job summary

NeighborHealth in Boston is seeking a compassionate Registered Nurse (RN) Care Manager to join our Project SHINE clinical care management team. This role involves coordinating care for individuals living with HIV/AIDS and requires flexibility to meet community needs. Essential duties include patient evaluations, treatment follow-ups, and development of individualized care plans.

The position requires at least a high school diploma, nursing licensure in Massachusetts, and offers various benefits including medical coverage and a starting pay range of $44.00 to $64.00 per hour based on experience.

Qualifications

  • Completion of vocational/technical training beyond high school in an accredited nursing program required.
  • Licensure as a Massachusetts R.N. is mandatory.
  • BCLS certification is required per departmental policy.

Responsibilities

  • Coordinate treatment of all STI consults and management.
  • Evaluate and follow up on HIV patients and PrEP/Npep treatment.
  • Develop individualized patient care plans in collaboration with the Project Shine team.

Job description

NeighborHealth, formerly East Boston Neighborhood Health Center, is one of the largest community health centers in the country, serving the greater Boston area.

Position Summary

As an integral member of the Project SHINE clinical care management team, the Registered Nurse (RN) Care Manager will have the opportunity to make a profound impact on the lives of people living with HIV/AIDS and people at risk for HIV transmission. The role requires flexibility, the ability to work with high‑need communities and may vary day‑to‑day to meet members where they are. Outreach methods may vary based on organizational needs and may include telephonic or in‑person interactions.

Schedule

Mon, Tues, Wed, & Fri 8:00 AM - 12:00 PM, & Thurs 1:00 PM - 5:00 PM with flexibility based on departmental needs.

Essential Duties & Responsibilities
  • Coordinate treatment of all STI consults, results, and management (Syphilis, Gonorrhea, chlamydia, etc.).
  • Evaluate and follow up on all patients receiving treatment for HIV, PrEP, and Npep.
  • Provide specialized nursing support for medical providers, interpreting STI lab results and assisting with HIV diagnosis and linkage to care.
  • Complete nursing intakes by assessing patients, obtaining histories, and performing appropriate tests and observations.
  • Provide comprehensive care, addressing care gaps, vaccinations, and annual screenings.
  • Develop individualized patient care plans, including adherence and outreach strategies, in collaboration with the Project Shine team.
  • Assume a leadership role on the multidisciplinary care team.
  • Interact with patients and staff in a professional and respectful manner and maintain a positive attitude.
  • Safely administer medications, tests, and treatments as ordered and manage medication refills with provider authorization.
  • Document patient histories, assessments, plans, and treatments thoroughly and concisely.
  • Implement Domestic Violence and Abuse Screening policy.
  • Follow established departmental policies for environmental safety and infection control.
  • Participate in quality improvement related to nursing functions.
  • Attend required departmental activities and meetings.
  • Perform other duties as assigned.
Education
  • High school diploma required.
  • Completion of vocational/technical training beyond high school in an accredited nursing program required.
  • Licensure as a Massachusetts R.N.
  • BCLS certification (per departmental policy) required.
Experience
  • New graduates are welcome.
  • Two years experience in HIV nursing preferred, preferably in a community health setting.

Pay Range

Starting at $44.00 up to $64.00 depending on experience.
