Job description

Job Summary

We are seeking an HIV Nurse Case Manager to work on a multidisciplinary HIV primary care team. You will provide high quality nursing care that is patient‑centered, non‑judgmental, and harm‑reduction focused. The role includes HIV and other chronic disease management, care coordination, triage, and communication of the care plan across team members.

This position is funded until the end of March 2028, with the possibility of renewal.

Responsibilities
  • Provide outpatient primary‑care nursing, including medical assessments and triage for same‑day or scheduled visits. Conduct virtual, telephonic, or in‑person nurse assessments to evaluate patient health concerns and advise appropriately. Document patient information accurately and in a timely manner in the EHR.
  • Implement the patient‑centered plan of care, evaluate outcomes, and communicate the plan of care, patient status, and progress toward goals with the patient and care team members. Conduct intake assessments for new HIV team patients and facilitate rapid linkages to medical providers.
  • Administer vaccines, injectable medications (including long‑acting injectables for HIV), inhaled respiratory therapies, and provide wound assessment and wound care as indicated. Provide medication adherence assessments and counseling; deliver directly observed therapy when directed.
  • Provide chronic disease management support for HIV and other common co‑morbid conditions such as diabetes and hypertension. Identify structural barriers patients face in accessing care and medication adherence, and work with the interdisciplinary team to address those barriers (e.g., housing, transportation, drug treatment, food, clothing, benefits).
  • Coordinate care with integrated behavioral health staff, assisting medical providers in coordinating follow‑up related to medication for opioid use disorder and other substance use disorders. Coordinate preventative services for HIV patients including education, preventative medications, vaccinations, testing, and referrals.
  • Facilitate patient linkages with internal teams (office‑based addiction treatment team, harm reduction team, medical respite team) and external programs to optimize outcomes.
  • Promote a compassionate and therapeutic environment that is responsive to the unique patient population. Demonstrate initiative and commitment to continuously improve services and processes that positively impact patient care and organizational goals. Participate in training and orientation of new nursing staff as directed by supervisors.
  • Exhibit a positive attitude with patients and establish professional and respectful relationships with internal and external healthcare teams, agencies, and facilities. Participate in training and orientation of new nursing staff as directed by supervisors.
Qualifications
  • Licensed as a Registered Nurse in the Commonwealth of Massachusetts.
  • Bachelor’s degree in nursing preferred; 2 years of experience in an ambulatory/outpatient care environment required.
  • HIV primary care experience highly preferred. Certification as an AIDS‑Certified Registered Nurse (ACRN) preferred; program funding is available to support this training after hire.
  • Expertise in substance use disorder and harm‑reduction principles strongly preferred.
  • Experience working with people experiencing homelessness strongly preferred; mental health experience, including knowledge of trauma‑informed care principles, preferred.
  • Experience working in a community health center setting is a plus.
  • Bilingual in Spanish and English preferred; bilingual in Haitian Creole is a plus.
Compensation and Benefits
  • The hourly rate ranges from $40.00 to $58.00, based on years of experience.
