Job summary

Community Clinic in Springdale, AR, seeks an RN Care Manager to coordinate care for high-risk patients in a primary and specialty clinic. This full-time, in-person role emphasizes self-management support, care planning, and transition-time case management.

The ideal candidate will have an Arkansas RN license, 2+ years in care management or utilization review, and strong knowledge of ambulatory care settings, HIPAA compliance, and EMR systems. Monday–Friday, 8:00 AM–5:00 PM, on-site position.

Qualifications

• High school diploma or equivalent required.
• Arkansas RN License required.
• 2+ years of Case/Care Management experience preferred.
• 2+ years of utilization review/case management experience preferred.
• 1 year of discharge planning/hospital or primary care experience preferred.
• Knowledge of ambulatory family practice setting.

Responsibilities

• Provides case management/utilization review and planning to assure patient progresses through the continuum of care.
• Coordinates integration of social services into patient care.
• Follows population health management and chronic disease protocols focusing on patient activation and care coordination.
• Ensures compliance with quality patient care and regulatory requirements.
• Collaborates with clinical staff in plan of care development and goal achievement.
• Enhances collaboration to maximize patient and family decision-making capacity.
• Ensures appropriate tests and labs are performed within established timeframes and results are promptly available.
• Promotes efficient utilization of clinical resources.
