• Formulating patient care management plans and setting treatment and self-management goals.
• Evaluating patient needs and coordinating care across services.
• Educating patients, visiting, and communicating to support adherence to care plans.
• Identifying resources to meet medical and non-medical needs of patients and families.
• Collaborating with programs to ensure proper coordination of services.
• Proactively identifying issues and proposing solutions for care coordination.
