Job summary

Responsibilities include assessments, care coordination, visits, and ongoing monitoring, with travel as required by state contracts. Strong EMR/HIPAA knowledge and independent work style are essential.

Qualifications

• Active RN license in the state of practice.
• Minimum 2 years in health care or care management.
• Valid driver's license and reliable transportation for travel requirements.
• Knowledge of EMR systems and HIPAA compliance.

Responsibilities

• Completes comprehensive assessments of members and determines care management eligibility.
• Develops and implements care coordination plans with member, caregiver and health professionals.
• Conducts telephonic, in-person or home visits as required.
• Monitors care plan effectiveness and documents outcomes.
• Maintains ongoing member caseload and outreach.
• Promotes integrated services and ICT collaboration.
• Facilitates interdisciplinary care team meetings and coordination.
• Uses motivational interviewing to support change.
• Identifies barriers to care and provides assistance.
• May provide consultations and resources to peers.
• Local travel 25-40% may be required.
