LanceSoft is seeking an experienced RN Care Manager in Kansas to coordinate patient care, discharge planning, and seamless care transitions. You will work with physicians, nurses, and multidisciplinary teams to ensure high-quality, patient-centered outcomes.

The ideal candidate holds an Associate Degree in Nursing (BSN preferred), an active Kansas RN license, and at least 2 years of acute Med-Surg experience. Strong communication and care coordination are essential for success.

Qualifications

• Active Kansas RN license
• Minimum 2 years of acute care Med-Surg nursing experience
• Associate Degree in Nursing or Diploma in Nursing (BSN preferred)
