Qualifications

Responsibilities

• Coordinate continuum of care from pre-admission to post-discharge.
• Liaise between patient, family, nurse, physicians and the care team.
• Lead interdisciplinary care plans and ensure timely resource use.
• Monitor admission, discharge planning and length of stay.
• Ensure regulatory and accreditation data collection.
• Perform other duties as assigned.
