Summary of Job Duties:

The RN Care Manager is responsible for managing the continuum of care from admission through discharge for assigned patients. The role requires knowledge of RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, Patient Bill of Rights, AB 1203, and other relevant regulations. The RN Care Manager collaborates with the multidisciplinary team, uses evidence-based practices, relevant data, and complies with UAMS's mission and core values.

*Hybrid remote staffing model may be available, but not required. Remote work depends on departmental staffing needs.

Qualifications:
Minimum Qualifications:
  • Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
  • Three (3) years RN experience with current BLS certification
  • Valid Arkansas Registered Nursing License
Preferred Qualifications:
  • National Nursing Certification
  • BSN (preferred); Certified Case Manager (CCM) or American Case Management (ACM) certification
Additional Responsibilities/Competencies:

Nursing Process:
  • Initiates appropriate referrals to internal and external teams.
  • Establishes a working diagnosis at initial review.
  • Communicates estimated discharge length and coordinates discharge planning.
  • Documents all communication and concerns.
  • Communicates with physicians regarding care levels and admission status.
  • Identifies and intervenes to ensure timely discharges, documents avoidable and denied days.
  • Refers psychosocial and non-medical discharge needs to Social Work.
  • Assesses discharge plans and revises as necessary.
  • Refers cases for Ethics Committee consideration.
Education/EBP/Research/QI:
  • Participates in Performance Improvement and Evidence-Based Practice.
  • Supports current evidence integration into practice.
  • Promotes educational environment for staff and students.
  • Provides patient and family education.
Communication/Collaboration:
  • Implements POC with Patient and Family Centered Care approach.
  • Collaborates effectively with multidisciplinary teams.
  • Documents care appropriately.
  • Reports variances through proper channels.
Leadership:
  • Acts as a resource to others.
  • Supports professional development of staff.
  • Participates in staff recruitment and retention.
  • Develops and implements evidence-based standards.
