Job summary

EGA Associates, LLC is seeking a qualified Registered Nurse Care Manager to support a Patient Aligned Care Team at a Primary Care Community Based Outpatient Clinic serving Veterans around Pottsville and Schuylkill County, PA. The clinic provides outpatient primary care in a community setting.

The RN will develop care plans, coordinate care, and provide nursing services within the PACT model, ensuring quality patient care and adherence to VA processes.

Qualifications

• Graduate of an accredited professional nursing program (ACEN/CCNE).
• RN license: current, full, active, and unrestricted or compact license.
• BSL certification or ability to obtain before patient care.
• Ability to complete VA and EGA credentialing and onboarding.
• Knowledge of care management, coordination, chronic disease management, population health, and outpatient operations.
• Ability to work in an interdisciplinary, team-based model of care.
• Strong verbal, written, clinical documentation, and patient-education skills.
• Ability to use EHRs, telehealth platforms, and standard computer apps.

Responsibilities

• Provide professional nursing services per licensure and standards.

Job SummaryEGA Associates is seeking a qualified Registered Nurse Care Manager to support a Patient Aligned Care Team at a Primary Care Community Based Outpatient Clinic serving Veterans primarily residing in and around Pottsville and Schuylkill County, Pennsylvania. The clinic provides primary care services in an outpatient clinical environment.

Essential Functions

• Provide professional nursing services to assigned and unassigned patients consistent with licensure, certification, nursing standards of practice, and the approved functional statement.
• Develop, oversee, and manage care management plans and care coordination in collaboration with Patient Aligned Care Team staff.
• Participate in secure messaging, virtual care, view-alert management, shared medical appointments, telephone care, and face-to-face visits.
• Identify patient needs for discipline-specific team members and discuss nursing recommendations with the Primary Care Provider.
• Engage appropriate PACT staff and enter consultation requests when required for formal communication.
• Delegate appropriate activities to qualified clinical or administrative associates while retaining accountability for assignments and outcomes.
• Enter orders for tests under approved standardized RN care-management protocols or Primary Care Provider orders.
• Provide same-day access for face-to-face and telephone nursing care.
• Use evidence-based guidelines, standardized nursing protocols, and accepted practice standards to promote patient engagement, self-care, and wellness.
• Manage patients transitioning between levels of care and participate in quality and performance-improvement activities.
• Perform additional duties as assigned.

Qualifications

• Graduate of a professional nursing program accredited by the Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education at the time of completion.
• Current, full, active, and unrestricted registration as a registered nurse in state where clinic is located or an acceptable multistate compact license.
• Current Basic Life Support certification or ability to obtain certification before beginning patient care.
• Ability to complete all required VA and EGA credentialing, background screening, and onboarding requirements.
• Knowledge of care management, care coordination, chronic disease management, population health, and outpatient nursing operations.
• Ability to work effectively within an interdisciplinary, team-based model of care.
• Strong verbal, written, clinical documentation, and patient-education skills.
• Ability to use electronic health records, telehealth platforms, and standard computer applications.

Preferred

• Previous primary care or ambulatory care nursing experience.
• Previous experience providing care to Veterans or working in a VA medical center, VA outpatient clinic, or contracted VA Community Based Outpatient Clinic.
• Experience with the VA Patient Aligned Care Team model, motivational interviewing, secure messaging, virtual care, or transitions of care.

Supervisory Responsibility

This position does not have direct employment supervisory responsibility. The RN Care Manager may delegate appropriate care activities to qualified clinical and administrative associates and remains accountable for the appropriateness of those assignments.

Travel

Travel is expected to be negligible unless required for orientation, training, meetings, or other contract-related activities.

Expected Hours of Work

Your work schedule may vary depending on the contract with the Client. Your start time should be discussed and approved by your facility supervisor if you wish to deviate from the normal operating hours. Employees may be required to work additional hours in order to successfully complete work obligations. Employees are required to obtain prior approval from their facility supervisor before working overtime.
