As an RN Care Manager for Behavioral Health Service, you’ll be a pivotal part of this mission. You'll lead the charge in improving healthcare quality for high-acuity and at-risk patients, crafting tailored care plans that make a real difference in their lives. Responsibilities

What You’ll Do:

  • Championing Care: You’ll identify patients in need, work hand-in-hand with them to develop personalized care plans, and guide them every step of the way to improve their health and wellbeing.
  • Collaboration is Key: Teamwork is at the heart of everything we do. You’ll collaborate with doctors, specialists, and community organizations to ensure each patient receives the best care, both in and outside the clinic.
  • Empowering Patients: You’ll engage patients in shared decision-making, helping them understand their options, and empowering them to take charge of their health.
  • Beyond the Clinic: From improving health literacy to connecting patients with valuable community resources, you’ll help reduce hospital visits and keep people healthy where they live, work, and play.
  • Data-Driven Decisions: You’ll dive into electronic health records to track patient data, analyze trends, and find ways to continuously improve care processes, ensuring everyone gets the right treatment at the right time.
  • Spreading the Word: You’ll also help promote CHC’s programs to the community, ensuring people know about the amazing services we offer.

Qualifications

What We’re Looking For:

  • A Registered Nurse with a passion for community health—BSN preferred
  • A data-savvy, team-oriented problem solver—you’ll be analyzing trends and improving workflows to make sure we’re always on top of our game
  • Experience in population health or care coordination (preferred)
  • A true communicator—both in person and digitally. Excellent verbal, written, and interpersonal skills are a must.
  • Fluency in Spanish and/or French Creole (preferred)
  • A reliable partner—a valid NJ driver’s license is required since you’ll be traveling to our various CHC locations.
