Job summary

Endeavor Health is seeking an RN Care Manager in Chicago, IL for a part-time position of 20 hours per week, with two 10-hour weekend shifts. The role focuses on comprehensive patient assessments and care coordination across the healthcare continuum.

The RN will collaborate with clinicians to tailor care plans, address barriers, and ensure timely referrals while maintaining HIPAA compliance and accurate EHR documentation.

Qualifications

• Bachelor's degree in nursing required.
• Master's degree in nursing preferred.
• 2 years of clinical nursing experience in acute care or hospital setting.
• IDFPR RN license in Illinois required.
• CCM certification preferred.
• CPR/BLS for healthcare provider (AHA).

Responsibilities

• Conduct thorough assessments of patients' medical, social, and psychological needs.
• Collaborate with healthcare providers to develop comprehensive care plans.
• Identify and address barriers to care and coordinate services accordingly.
• Serve as a liaison between patients, families, providers, and community resources.
• Monitor and manage patient progress through the healthcare continuum.
• Coordinate referrals to specialists and arrange home healthcare services.
• Oversee transitions of care including admissions and discharges.
• Provide information about diagnoses, treatment options, and support resources.
• Educate patients on self-management and engage them in care decisions.
• Maintain accurate electronic health records of interactions and interventions.
• Prepare and present reports on patient progress to the healthcare team.
• Collaborate to improve care processes and outcomes.
• Participate in quality improvement initiatives and adhere to HIPAA.

Hourly Pay Range:

$40.45 - $62.70 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

What you will do:

• Collaborate with the interdisciplinary team to identify opportunities for improving care processes and patient outcomes.


• Participate in quality improvement initiatives and contribute to the development of best practices.


• Ensure compliance with all relevant healthcare regulations and standards, including HIPAA.

What you will need:

• Bachelors Degree Nursing Required Or


• Masters Degree Nursing Preferred


• 2 Years of clinical nursing experience, preferably in an acute care or hospital setting.


• Registered Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) Current Registered Nurse (RN) license in the state of Illinois. Required And


• Certified Case Manager (CCM®) - Commission for Case Manager Certification (CCMC) Certification in Case Management (CCM) or related field Preferred


• CPR/BLS for the healthcare provider American Heart Association (AHA)
