You will ensure regulatory compliance, optimize discharge planning, and provide strong leadership to the team to deliver patient-centered care and achieve optimal outcomes. Qualifications

• Bachelor's Degree required or 7+ years direct experience in lieu of degree.
• Master's Degree preferred.
• 3-5 years of experience in closely related field with Bachelor's degree required.
• 3-5 years of leadership experience preferred.

Responsabilités

• Oversee the implementation and maintenance of a compliant Care Management program.
• Provide leadership to staff and ensure accountability for duties.
• Conduct chart reviews to evaluate severity of illness and intensity of services.
• Coordinate discharge planning and social services to ensure regulatory compliance.
• Collaborate with leadership to optimize quality and efficiency in patient care.
