PeaceHealth is seeking an experienced RN Care Manager to join our collaborative team and play a vital role in helping patients navigate their healthcare journey.

You'll work closely with physicians, patients, families, and interdisciplinary teams to improve outcomes, enhance the patient experience, and make a meaningful impact every day.

What You'll Do
- Coordinating and facilitating individualized discharge plans across the continuum of care
- Assessing patients' clinical, psychosocial, environmental, and financial needs
- Collaborating with physicians, nursing staff, therapists, and community resources to ensure smooth transitions of care
- Identifying barriers to discharge and proactively developing solutions
- Conducting utilization reviews to ensure appropriate level-of-care determinations
- Partnering with payors and patient financial services to support authorization and reimbursement processes
- Facilitating interdisciplinary care team meetings focused on quality, efficiency, and patient satisfaction
- Supporting patients and families through complex healthcare decisions and care planning
- Maintaining accurate and timely documentation in accordance with regulatory and organizational requirements
What We're Looking For
Required Qualifications
- Graduate of an accredited School of Nursing
- Current Registered Nurse (RN) license in the state of practice
- Basic Life Support (BLS) certification upon hire
- Minimum three years of clinical nursing experience in an acute care hospital setting
Equivalent experience in skilled nursing, outpatient case management, home health, or hospice may also be considered.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Master of Science in Nursing (MSN)
- Utilization Management or Quality Management experience
- Specialty certification
- Knowledge of third-party reimbursement and healthcare regulations
