Job summary

PeaceHealth is seeking an experienced RN Care Manager to join our collaborative team in Washington. The role is full-time at 0.90 FTE with daytime hours, focusing on coordinating care transitions and patient advocacy across the continuum.

You will work with physicians, families, and interdisciplinary teams to improve outcomes, while utilizing clinical judgment to ensure appropriate levels of care and documentation in line with regulatory standards.

Qualifications

• Graduate of an accredited School of Nursing.
• Current RN license in the state of practice.
• BLS certification upon hire.
• Minimum three years of clinical nursing experience in an acute care setting.

Responsibilities

• Coordinating and facilitating discharge plans across the continuum of care.
• Assessing clinical, psychosocial, environmental, and financial needs.
• Collaborating with physicians, nursing staff, therapists, and community resources for smooth transitions of care.
• Identifying discharge barriers and developing solutions.
• Conducting utilization reviews for appropriate level of care.
• Partnering with payors and patient financial services for authorization and reimbursement.
• Facilitating interdisciplinary care team meetings focused on quality, efficiency, and patient satisfaction.
• Supporting patients and families through complex healthcare decisions and care planning.
• Maintaining accurate documentation per regulatory requirements.

What You'll Do

• Supporting patients and families through complex healthcare decisions and care planning
• Maintaining accurate and timely documentation in accordance with regulatory and organizational requirements

What We're Looking For

Required Qualifications

• Graduate of an accredited School of Nursing
• Current Registered Nurse (RN) license in the state of practice
• Basic Life Support (BLS) certification upon hire
• Minimum three years of clinical nursing experience in an acute care hospital setting

Equivalent experience in skilled nursing, outpatient case management, home health, or hospice may also be considered.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Master of Science in Nursing (MSN)
• Utilization Management or Quality Management experience
• Specialty certification
• Knowledge of third-party reimbursement and healthcare regulations
