RN Care Coordinator Needed for Remote Contract Opportunity

Location: Philadelphia, PA (Remote, all eligible candidates must reside in the Tri-state area)
Schedule: M-F 8am-5pm
Duration: 3 months with possible extension or direct placement
Pay: $31-38/hr DOE

Responsibilities:

• Apply critical thinking and medical knowledge to cases, making determinations based on resources and guidelines.
• Use behavioral health criteria to establish needs for inpatient, continued stay, and services.
• Communicate with providers regarding treatment plans and clarify medical needs.
• Review treatment plans with providers, clarifying behavioral health information as needed.
• Refer cases not meeting criteria to the Medical Director.
• Identify discharge planning needs early and collaborate with staff to facilitate appropriate discharges.
• Report utilization issues and trends, and refer cases to relevant departments.
• Ensure requests are covered within the member’s benefit plan and comply with regulations.
• Meet turnaround time and productivity goals, and document all functions accurately.

Qualifications:

• RN, LSW, LCSW, LPC, or LMFT licensure valid in PA, NJ, or DE
• Residency in the tri-state area
• 3+ years of Behavioral Health clinical experience
• Experience in utilization management is preferred.
