Ohio State University Physicians seeks a Clinical Nurse Care Coordinator to partner with clinic teams, delivering evidence-based, patient-centered care. You’ll help patients stay current with care plans and goal attainment, with emphasis on high-risk medically complex cases and care transitions.

You’ll liaise with families and other agencies, ensure communication, and stay current with standards while supporting programs and processes in a collaborative, compassionate environment.

Qualifications

• Graduate-nurse education required: Associate Degree in Nursing from CCNE/ACEN accredited program.
• Current Ohio RN license.
• CPR/BLS certification required.

Responsibilities

• Collaborate with clinic teams to deliver evidence-based, patient-centered care.
• Focus on high-risk medically complex patients and care transitions.
• Serve as liaison with patient families and other agencies to improve communication.
• Stay up to date on standards and support program/process improvements.
