St. Elizabeth Healthcare is seeking a Registered Nurse for a Care Coordination role at our Fort Thomas, Kentucky hospital. The position is full-time, 32 hours per week, 1st shift.

The Care Coordinator works with patients, families, social workers, and physicians to ensure efficient progression through the continuum of care, optimizing transitions and care planning to maximize outcomes.

Qualifications

• Bachelor's degree in nursing from an accredited school.
• Compact (multi-state) license in primary state of residence (if eligible).
• RN licensed in the state where work is performed.
• Knowledge of state and federal regulatory issues, payor strategies, and benefit plan designs.
• Familiarity with SEHC policies and procedures.
• Excellent organizational, problem-solving and communication skills.
• 2 years' experience in acute care.
• FLSA Status: Exempt

Responsibilities

• Identify and manage optimum patient flow to enhance continuity of care.
• Plan and facilitate coordinated transitions to the next level of care.
• Promote patient satisfaction and quality outcomes.
• Target an optimal length of stay based on patient response and available services.
• Provide care management planning to ensure quality patient care and regulatory compliance.
• Demonstrate respect, dignity and empathy in all encounters.
