They act as consultants and educators for healthcare teams, focusing on discharge planning, resource coordination, and system improvement to optimize resource utilization while maintaining quality care. The role involves managing complex cases, coordinating post-discharge care, and facilitating safe patient transitions by communicating with patients, families, and interdisciplinary teams. Responsibilities include ensuring compliance, thorough documentation, and addressing barriers to safe transfers.
Qualifications: Minimum two years RN experience across Med/Surg, ICU, ED, Home Health, Skilled Nursing, Hospice, or Rehab. Valid RN license, BLS certification, and multi-state credentials are required. Skills include effective communication, teamwork, assessment, documentation, problem-solving, and culturally competent care. Preferred qualifications include knowledge of utilization management, Medicare/Medicaid regulations, and certification in Case Management, with a BSN or MSN being advantageous.
