Your Health Organization in the South Carolina and Georgia area seeks a Registered Nurse to join our urgent care family. The role focuses on care coordination across homes, facilities, and clinics, with collaboration with the provider and hospice teams.

Essential duties include admissions, coordinating with care teams, telehealth facilitation, and maintaining EMR records. Leadership, communication, and strong interpersonal skills are required to support quality patient outcomes.

Qualifications

• Must be a licensed nurse.
• License must be in good standing with the appropriate board.
• Minimum of three (3) years clinical experience preferred.
• Demonstrated ability to supervise and direct personnel.
• Ability to read and communicate effectively.
• Ability to manage and demonstrate effective leadership skills.

Responsabilités

• Admission
• Communication, collaboration, and coordination of care
• Coordinate with the patient health care team, physicians, therapists, home health or hospice representatives
• Facilitate a telehealth visit with the nurse practitioner for coordination of care
• Coordinate with hospice interdisciplinary team and other care plan participants
• Maintain current list of providers and specialists updated in the EMR
• Communicate with patients, families, and caregivers
• Participate in coaching calls
