Job Summary

Reports to the RN Manager of Care Coordination, the RN Care Coordinator (OCC) works collaboratively with providers, interdisciplinary staff, and clinical associates, in person and telephonically, at any/all SEP offices to support patients with chronic conditions and/or complex needs according to guidelines established by SEP and other clinical programs such as PCF. Facilitates effective communication, coordinates services, addresses barriers, and provides education and guidance for patients related to current health concerns.

Job Responsibilities
  • Documents in chart appropriately utilizing care management documentation.
  • Provides patient care through collaborating with patients, providing education and clear directions, and addressing patient concerns regarding care.
  • Supports Chronic Disease Management and Patient Care Needs, including identifying patients with chronic diseases, responding to provider referrals, evaluating patient readiness, monitoring compliance, and providing support and education.
  • Performs assessments such as ADL and home safety, and orders services like home health when needed.
  • Utilizes teach-back methods and provides diabetic teaching as appropriate.
  • Documents interventions in Epic and coordinates referrals to community resources.
  • Assists with advance care planning and explains screening results based on protocols.
  • Performs medication reconciliation and manages ongoing chronic condition care.
  • Provides patient education, both one-on-one and group sessions, and refers patients to appropriate specialists and resources.
  • Ensures complete and accurate documentation, maintains resources, and complies with OSHA and HIPAA standards.
  • Supports care transitions, participates in meetings, and performs in-office nursing tasks as needed.
Required Skills and Knowledge
  • Ability to manage multiple tasks and prioritize effectively.
  • Proficiency with electronic Health Records (EPIC), Microsoft Office Suite.
  • Strong organizational, communication, and interpersonal skills.
  • Knowledge of motivational interviewing and critical analysis skills.
  • Self-motivated and able to work both autonomously and collaboratively.
Other Skills and Education
  • Previous QA experience preferred.
  • Degree in nursing (ADN or higher), current Kentucky RN license, and reliable transportation.
  • Care Management Certification preferred.
Experience
  • Minimum 3 years nursing experience or current care management role within SEP.
  • Knowledge of anatomy, physiology, pharmacology, and ambulatory care.
