Job summary

Qualifications

• Minimum 2 years of acute hospital patient care experience.
• UR/QA experience or utilization management experience is preferred.
• Proficient with electronic medical records and health information systems.

Responsibilities

• Coordinate utilization management and care transitions for hospitalized patients.
• Assist with discharge planning and safe transitions to post-acute care settings.
• Collaborate with interdisciplinary care teams to optimize resource use and patient outcomes.
• Provide patient and family education regarding care plans, medications, and follow-up.
• Generate and review reports related to patient care and discharge processes.
• Support denial/appeal workflows and communication with payers as needed.
• Maintain knowledge of regulatory standards and hospital policies.
