Job summary

Saint Luke’s South is seeking a Nurse Care Coordinator (RN) to join our team in Overland Park, KS. The role focuses on care transitions, discharge planning, and collaboration with the multidisciplinary team to ensure quality outcomes.

The position requires a BSN to be completed within 3 years of start and at least 1 year of RN experience, with BSN preferred. Schedule is full-time days, with typical inpatient care coordination responsibilities across the continuum.

Qualifications

• RN license required.

• 1 year applicable nursing experience.

Responsibilities

• Assess needs and develop individualized discharge plan.
• Identify additional services needed and make appropriate referrals.
• Lead multidisciplinary rounds and ensure clinical readiness for discharge.
• Collaborate with the team to mitigate delays in care and manage throughput.
• Identify appropriate length of stay and monitor resource utilization with physician oversight.
• Maintain thorough documentation of patient care and discharge plans.
