Saint Luke’s South is seeking a Nurse Care Coordinator (RN) to oversee care transitions and discharge planning. This full-time, day-shift role focuses on coordinating care across the care continuum and ensuring timely discharges. BSN preferred within 3 years of start.

Qualifications

• 1 year applicable nursing experience
• BLS certification (American Heart Association or Red Cross)
• Bachelor’s Degree in Nursing or BSN required (within 3 years)

Responsibilities

• Assess needs and develop individualized discharge plans
• Identify additional services and referrals
• Lead and facilitate multi-disciplinary rounds and ensure clinical readiness for discharge
• Collaborate with the multidisciplinary team to mitigate delays in care
• Identify appropriate length of stay and monitor resource utilization
• Coordinate with patients and families for timely discharge and proper documentation
