Job summary

Sarah Bush Lincoln is seeking an RN for Care Coordination to coordinate team-based care across settings, partnering with patients, families, and physicians to achieve high-quality, patient-centered outcomes.

The role is three days per week from 8:00am to 4:30pm, with pay based on experience starting at $32.10 per hour and a compensation range up to $49.76 per hour, depending on experience. Career development and on-site training are provided.

Qualifications

• ADN required.
• BLS or CPR certification within 90 days of hire.

Responsibilities

• Actively participates in data collection and analysis for ACO.
• Assess patient and family’s unmet health and social needs.
• Coach patients/families toward successful self-management of chronic disease.
• Coordinate a strategic approach to detect and manage patients with chronic disease.
• Establish an efficient internal tracking system for identified patients.
• Facilitate care coordination with providers, payers and community resources.
