Job summary

The U.S. Department of Veterans Affairs is seeking a Care Coordinator Registered Nurse (RN) in the Specialty service at the West Haven VA Connecticut Healthcare Systems in West Haven, CT.

The role is full-time (40 hours/week) and focuses on patient management, education, and discharge planning in an outpatient/surgical context. Candidates should have 3–5 years nursing experience, an active RN license, and demonstrated care coordination/case management skills.

Qualifications

• English language proficiency per VA requirements.
• Graduate of an ACEN or CCNE accredited nursing program.
• Current, full, active RN license in US or territories.
• GNT option available temporarily if licensure is pending.
• Preferred 3–5 years nursing experience; care coordination/case management valued.
• Refer to VA Qualification Standards for nurse progression.

Responsibilities

• Provide care coordination, discharge planning, and patient management in outpatient settings.
• Prepare patients for procedures and assist with pre-procedure assessments.
• Educate patients and families on care plans and post-procedure needs.
• Collaborate with Cath Lab staff, anesthesia, and interdisciplinary teams.
• Maintain clinic performance metrics and participate in quality improvement.
• Work schedule: full-time, day shift; telework ad-hoc per agency policy.
