Job summary

Cleveland Clinic is seeking an RN Care Coordinator for Asthma in the Pulmonary Medicine department. The role focuses on coordinating care for high‑risk asthma patients across outpatient and inpatient settings.

The position offers a dayshift, Monday–Friday schedule with potential for one day of remote work per week, and opportunities to improve patient outcomes through education and care planning. Prior RN and BLS credentials required.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state RN licensure.
• BLS certification through AHA or Red Cross.
• Three to five years of nursing experience.
• Strong attention to detail and therapeutic communication skills.

Responsibilities

• Collaborate with multidisciplinary care teams to develop goals and plan interventions.
• Coordinate services and education to patients to reduce ED visits and readmissions.
• Utilize registries and patient lists to manage a defined panel of high-risk patients.
• Provide disease management coordination and patient/family education.
• Assist in transitions of care across settings with clear communication.
