Qualifications

• At least one year of recent Case Management experience in an acute care setting.
• Current CA Licensure.
• Current CPR recognition.

Responsibilities

• Coordinate care and services for assigned patients within an episode of illness or treatment.
• Collaborate with patient, family, physicians, care team, and payors to optimize care.
• Utilize clinical skills to assess, plan, intervene and evaluate nursing care.
• Promote efficient use of resources and lead communication among providers.
• Participate in quality/utilization reviews and disposition decisions as needed.
• May be assigned to the Emergency Department to support care coordination.
