Job summary

Keystone Health in Chambersburg, PA seeks an experienced RN Care Coordinator to manage patient care plans and transitions. This full-time, Monday–Friday role focuses on chronic disease management, care planning, closing gaps, and connecting patients with community resources.

You will educate patients and families, collaborate with providers and care teams, and contribute to improving health outcomes and patient experience.

Qualifications

• Associate degree in Nursing required; Bachelor’s degree preferred.
• Current RN license and CPR certification required.
• Clinical nursing experience required; care management experience preferred.
