The LHCSA Regional Director / Director of Patient Services (DPS) supervises the provision of nursing services provided by the Licensed Agency in a selected region. This person is available at all times during operational hours and participates in activities with respect to professional services. Collaborates between Departments to facilitate and assure the delivery of high-quality client care.
Essential Functions
- Supervise the day-to-day clinical operations (including ensuring workflow tasks are completed even during staff absences) and all required client care is completed, including but not limited to:
- Review and follow-up on complaint forms and all QI reports.
- Onsite supervision of RN case managers.
- Oversee the instruction of nursing and other staff in providing quality service and maintaining standards, including in-home visits to direct and evaluate client care.
- Proactively address operational and clinical practices to prevent client harm and / or organizational deficiencies or inefficiencies.
- Assure that the client’s plan of care is executed as written and reassessed by the appropriate healthcare professional, especially when there is a significant health status change, at the physician’s request, or after hospital discharge.
- Recommend policies, procedures, and protocols based on industry regulations and evidence-based clinical practice standards.
- Monitor scheduling personnel’s effectiveness to ensure visits are assigned based on nursing competencies and workloads are equitable.
- Ensure staff perform according to Agency standards, legal/regulatory standards, and the Nurse Practice Act.
- Conduct case conferences to discuss client issues, plan of care, and discharge planning, documenting these via the Case Conference Coordination Note in the medical record.
- Collaborate with HR and the Executive Director in recruiting qualified personnel and developing staff retention strategies.
- Determine staffing levels and maintain adequate staffing to meet client requirements.
- Approve weekly schedules and ensure productivity standards are met.
- Review and approve bi-weekly payroll transmittal.
- Recommend and collaborate on in-service education programs to promote staff development.
- Maintain current nursing knowledge, aligned with community standards and best practices.
- Participate in community awareness programs as needed.
- Understand and interpret operational budgets and respond to variances.
- Create strategies to meet corporate goals, including attainable team objectives, and evaluate their effectiveness using measurable criteria.
- Hold monthly staff meetings to facilitate agency communication.
- Participate in mandatory agency trainings.
- Adhere to agency policies, procedures, and employee guidelines.
- Demonstrate nursing leadership competencies and emotional intelligence.
- Log onto the HealthCommerce site weekly for updates.
- Stay informed of federal and state regulations and understand contract terms.
- Perform other duties as assigned.
Qualifications and Requirements
- Bachelor of Science Degree in Nursing or equivalent home care experience.
- Current, valid RN license issued by NYS Department of Education.
- Minimum of two years’ experience in home care, with at least two years’ nursing experience within the last 36 months.
- Understanding of payors for home health services.
- Organized, detail-oriented, with effective communication skills both orally and in writing.
- Ability to problem-solve and prioritize multiple tasks effectively, even under stress.
- Ability to project the agency’s mission and translate organizational objectives into actionable strategies, reflecting core values.
Physical and Work Environment
The primary setting is an office, with possible outdoor conditions. The role is classified as sedentary work, involving sitting most of the time and exerting up to 10 pounds of force occasionally.
