Brighton Health Plan Solutions, LLC is seeking a Registered Nurse Case Manager to join our Case Management team in North Carolina. The role reports to Supervisor, Case Management and partners with Senior Director for clinical guidance.

You will perform comprehensive assessments, develop member-centered care plans, coordinate care across the continuum, and ensure HIPAA and NCQA standards are met. Bachelor's degree preferred; RN with compact licensure and 2+ years in case management, behavioral

Qualifications

• Active RN license required with multistate (compact) licensure.
• BSN preferred; ADN with relevant experience considered.
• Minimum 2 years in case management or related clinical setting.
• Experience in behavioral health case management is preferred.
• Proficiency with Microsoft Office and EMR/case management tools.

Responsibilities

• Conduct comprehensive assessments of medical, behavioral health, psychosocial, and functional needs.
• Develop, implement, and monitor member-centered care plans with interdisciplinary teams.
• Coordinate care across the healthcare continuum to improve outcomes and optimize utilization.
• Educate members and caregivers on health conditions, treatment plans, medications, and self-management.
• Identify barriers to care and opportunities to improve member outcomes.
• Document assessments, plans, interventions, and progress in accordance with standards.
• Maintain HIPAA compliance and NCQA Care Management standards.
• Participate in quality improvement activities and accreditation processes.
