Job summary

Santa Rosa is seeking a Nursing Care Coordinator to coordinate patient care across the health care continuum. Responsibilities include supervising management of patient care using nursing process, discharge planning, linking and scheduling patients with services, monitoring care delivery, advocating for patients, care planning and evaluating outcomes.

The role may require flexibility of hours to meet patient needs and coordinate with the care team.

Job description
