Job summary

Santa Rosa in Pasadena is seeking a clinical coordinator responsible for coordinating patient care to meet identified needs of patients and families. You will supervise care delivery, apply nursing processes, discharge planning, and linking patients with required services to achieve optimal outcomes.

In this role, you’ll work with the care team across the health care continuum, monitor care delivery, and support colleagues in addressing patient needs. Flexibility in hours may be required.
