Affinity is urgently seeking reliable RNs and LPNs to provide shift care for 2 hours per day in the 19132 zip code of Philadelphia. We are looking to staff this case as soon as possible.

Responsibilities

• Provide skilled nursing care for patients in the home
• Record patient medical history accurately
• Take and record vital signs (blood pressure, temperature, heart rate, etc.)
• Observe patients for progress, medication side effects, and changes in condition
• Monitor fluid intake/output and maintain accurate patient charts
• Administer medications, injections, wound care, and other basic medical tasks
• Collect samples for lab testing as assigned
• Communicate with PCP and other care team members as needed
• Provide emotional and psychological support to patients and families
• Offer guidance and instruction to family members when appropriate

Requirements

• Active LPN or RN license in Pennsylvania
• Experience with trach/vent care preferred (if applicable to the case)
• Ability to work independently and as part of a multidisciplinary team
• Strong communication and interpersonal skills
• Physically able to perform all nursing duties
• Compassionate, reliable, and able to manage stress in a home care environment
• Familiarity with Axxess or HHAeXchange is a plus
