Qualifications

• Current Registered Nurse (RN) license in state of practice.
• BSN within four years of hire or promotion.
• Experience in chronic disease management and familiarity with terminology.
• Proficiency in basic computer skills and Microsoft Office.

Responsibilities

• Identifies patients for proactive interventions and develops individualized care plans.
• Manages and monitors care plans, coordinating internal and external services.
• Supports patient self-management using coaching techniques.
• Educates healthcare team and patients about case management processes.
• Collaborates with physicians and care team to ensure timely care.
