A leading healthcare organization in New York seeks a dedicated RN to oversee patient care, coordinate with healthcare teams, and support patient advocacy. The role involves managing care plans, supervising staff, and ensuring effective communication within the care team.
Overview
Assumes responsibility and accountability for patient screenings, triage, pain management, falls prevention, nutritional and psychosocial interventions, discharge planning, and other measures to promote patients’ physical health. Utilizes the nursing process to provide direct patient care. Acts as a patient advocate and navigator through the healthcare system, assisting patients and families/significant others. Responsible for prescribing, delegating, and coordinating all care for assigned patients, executing medical plans of care under the direction of treating clinicians, in accordance with RN licensure. Uses nursing diagnoses to address human responses to health problems through case finding, health teaching, counseling, and supportive/restorative care. Supervises care provided by LPNs, MOAs, NAs, technicians, and staff. Supports patient self-management and behavior modification, coordinating care with external healthcare organizations from admission to discharge, including referrals. Ensures clear communication within the care team and participates in data collection, outcomes reporting, audits, and program evaluation related to Patient-Centered Medical Home and ambulatory care initiatives. Assists with prior authorization for clinical and pharmacy orders. Demonstrates teamwork in an interdisciplinary healthcare setting.
