Qualifications

The Registered Nurse will utilize a team-based approach to manage patient needs, providing care coordination, advocacy, coaching, and education for high-risk patients. The RN will assess, support, document, and collaborate with the healthcare team to deliver comprehensive care, focusing on both chronic and preventive health measures to improve quality outcomes.

Job Responsibilities:

  • Identify high-risk patients, establish relationships, and develop care plans with goals.
  • Address barriers to care, implement support interventions, and optimize access to services.
  • Coordinate effective transitions of care, including follow-up after hospital or nursing facility discharges.
  • Promote health maintenance and participate in community and care team collaborations.
  • Assess and triage incoming calls and walk-in patients.
  • Follow up on ED and urgent care visits.
  • Support population health initiatives focusing on prevention and chronic disease management.
  • Monitor patients on anticoagulation therapy.
  • Participate in outreach activities like vaccine clinics and direct patient care.
  • Provide culturally competent care tailored to patient needs.
  • Perform other duties as assigned.

Requirements:

  • Excellent communication skills and ability to build partnerships.
  • Knowledge of healthcare delivery in various settings; community resources preferred.
  • Ability to engage and prioritize patient needs.
  • Basic computer skills, including EHR proficiency.
  • Bilingual skills preferred but not required.
  • Flexible schedule, including some evenings and weekends.

Education/Experience:

  • Graduate of an approved nursing program with RN license in NYS.
  • 2-3 years’ experience in primary care, internal medicine, family medicine, public health, or case management preferred.
  • Current BLS certification or equivalent.

  • Attend meetings and ongoing training.
