Job summary

BrightStar Care is seeking an Assistant Director of Nursing (ADON) / RN Case Manager to support the Director of Nursing in coordinating clinical operations and ensuring quality patient care. You will supervise field staff, conduct client assessments, and develop care plans to promote positive patient outcomes.

This role requires Indiana RN licensure, 2+ years of nursing experience (home health preferred), and strong assessment, communication, and organizational skills.

Qualifications

• Current Indiana RN license in good standing.
• Minimum 2 years of nursing experience; home health experience preferred.
• Previous supervisory or case management experience preferred.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct client assessments, admissions, and reassessments.
• Develop and manage individualized Plans of Care.
• Supervise and support nurses, caregivers, and home health aides.
• Provide clinical guidance, training, and competency evaluations.
• Monitor documentation for accuracy and regulatory compliance.
• Coordinate care with physicians, families, and community resources.
• Assist with quality assurance, audits, and accreditation readiness.
• Support the Director of Nursing with daily clinical operations and staff management.
• Participate in on-call rotation as needed.
