Job summary

The Admissions RN at Riverside Health System in Newport News, VA supports smooth transitions of care by coordinating admissions with hospital discharge planners, reviewing charts and insurance, and ensuring readiness for admission.

You will collaborate with care teams to meet patient needs, provide emotional support to patients and families, and help maintain safety and quality of care during the admission process.

Qualifications

• 2 years of experience in long-term care or skilled nursing (preferred).
• RN licensed in Virginia or a compact state; active on hire.
• CPR/BLS certification required within 30 days.
• Valid driver's license required.
• Weekend shifts may be required.

Responsibilities

• Maintains working relationships with providers and other medical personnel to support quality clinical care.
• Coordinates with discharge planners and Central Admissions to complete admissions (chart review, insurance review, face sheet, follow-up).
• Evaluates admission readiness to minimize readmissions to the hospital environment.
• Participates in interdisciplinary Plan of Care with hospital.
• Assesses patient needs (services, medications, equipment) to ensure facility can provide appropriate care.
• Completes daily workload within scheduled shift and supports team.
