Job summary

Interim HealthCare Home Care in Port Matilda/State College area seeks a part-time Registered Nurse (RN) for 1:1 in-home care. The role delivers direct patient care at home under a prescribed plan to ensure comfort and dignity for clients.

The RN will assess conditions, document vitals, administer medications and IV fluids, manage wounds, and coordinate with health professionals to achieve quality patient outcomes.

Qualifications

• Licensure: Current unrestricted RN license in the state where the position is located.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites and/or work locations.
• Practical trach and/or ventilator experience preferred but not required.

Responsibilities

• Provide in-home direct patient care per prescribed plan of care and nursing scope.
• Assess and document patient conditions; monitor progress and vital signs.
• Administer medications, insulin, IV fluids; document thoroughly.
• Inspect and care for wounds; assist with personal hygiene and dressing changes.
• Assess for further injuries or complications and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure outcomes.

Status: Part-time, 20 hours a week

As a Registered Nurse(RN) you will

• Providein-homedirect patient care according to theprovider’sprescribed plan of care (and nursingscope of practice) while closely monitoring progress/recovery.
• Assessand document patient conditions and reportchangeas needed (vitals,blood/stool/urine, glucose,etc.)
• Administer medication, insulin, and IV fluids:documenting thoroughly.
• Inspect and care for wounds, changing dressings and assisting with personal hygiene.
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Registered Nurse(RN) position with us
