Job summary

Randstad USA in Lynn, MA is seeking a Registered Nurse to join the PACE Interdisciplinary Team on a full-time basis. You will participate in planning, implementing, and evaluating care plans to meet PACE model objectives and support frail elders remaining safely in the community.

The role requires MA RN licensure, at least 2 years in geriatric care, strong Microsoft Office skills, and the ability to travel locally.

Qualifications

• Registered Nurse licensed in the Commonwealth of MA.
• Minimum 2 years experience with geriatric population.
• Proficient with Microsoft Office tools.
• Ability to travel locally in own vehicle.
• COVID vaccinated preferred.
• Spanish speaking preferred.

Responsibilities

• Provides direct care to participants; completes initial and periodic assessments and revises care plans as needed.
• Collects essential medical and social information to support comprehensive decision‑making for participant safety and wellbeing.
• Monitors clinic care and service delivery to ensure adherence to quality assurance policies and procedures.
• Remains attentive to participants' health status and social functioning; identifies and prioritizes problems, handles immediate referrals or follow‑ups, and records diagnostic results.
• Coordinates with interdisciplinary team to develop comprehensive care plans reflecting participant and family perspectives.
• Takes part in summarizing each client to help determine Nursing Home level of care.
• Ensures all tests, lab work and diagnostic studies ordered by the physician are completed.

Location: Lynn, MA, US

Salary: $80,000.00 - $104,000.00 annually

Hours: M-F 8am-4pm, no weekend or holiday hours
