Job summary

Hillcrest Health & Living is seeking a compassionate Hospice RN for Hillcrest Hospice in Lincoln, NE. The role provides skilled nursing care, coordinates patient plans, and supports families through end-of-life care.

Hillcrest Health & Living is looking for a compassionate, clinically strong, and dependable Hospice RN to join our Hillcrest Hospice team.

What You’ll Do

As a Hospice RN, you will provide direct patient care while managing and coordinating each patient’s plan of care. You will use strong clinical judgment, communication, and problem-solving skills to support patients, families, caregivers, physicians, and the interdisciplinary hospice team.

You Will
  • Provide nursing visits and assess patients’ physical, functional, psychosocial, spiritual, cultural, environmental, and nutritional needs.
  • Manage patient care with a focus on bedside care, symptom management, crisis intervention, and family support.
  • Coordinate and integrate services provided by all hospice disciplines.
  • Teach patients and families about the disease process, self-care techniques, end-of-life care, and ethical concerns.
  • Communicate professionally with physicians, patients, families, caregivers, referral sources, and team members to support the plan of care.
  • Notify patients and caregivers of schedules and schedule changes as needed.
  • Communicate changes in patient condition or care needs to leadership and appropriate team members.
  • Provide nursing services in accordance with each patient’s plan of care.
  • Prepare accurate clinical notes, progress notes, assessments, and care plan updates.
  • Complete documentation at the point of care and follow hospice documentation standards.
  • Initiate, document, and revise hospice aide care plans based on patient condition.
  • Track visit time, travel time, meetings, mileage, and related daily documentation accurately.
  • Prepare clinical documentation for interdisciplinary team meetings and communicate recommended interventions.
  • Participate in team conferences, service line meetings, QAPI activities, committees, and ongoing education.
  • Provide consultation, education, and support to internal and external stakeholders.
  • Provide specialized hospice education to staff, family members, and informal caregivers as needed.
  • Participate in on-call and backup call rotations as needed to support patient care.
  • Travel to multiple work locations to provide patient care and support hospice needs.

The right candidate will be clinically confident, self-directed, and able to build strong relationships with patients, families, caregivers, providers, referral sources, and team members. Hospice care requires heart, follow-through, strong communication, and the ability to support families with both clinical skill and genuine compassion.

Ideal Candidates Will Have
  • At least three years of nursing experience required.
  • At least two years of hospice, home care, palliative care, or related experience preferred.
  • Strong clinical assessment, symptom-management, and care-coordination skills.
  • Excellent written and verbal communication skills.
  • Strong problem-solving and decision‑making abilities.

  • Strong attention to detail with documentation, care planning, schedules, mileage, and follow‑through.
  • A calm, flexible, and professional approach in changing or urgent situations.
  • Comfort educating patients, families, caregivers, and team members.
  • Willingness to travel to multiple work locations.
  • A compassionate, patient‑centered approach to end‑of‑life care.
Education & Credentials
  • Graduate of an accredited nursing program required.
  • Active Registered Nurse license in Nebraska, Iowa, or a compact state required.
  • Bachelor’s degree preferred.
  • Valid driver’s license required.
  • Reliable transportation and current automobile insurance required.
  • Basic Life Support certification through the American Heart Association required.
