Job summary

Northshore Home Health Care seeks a physician leader to drive enterprise risk adjustment strategy and clinical documentation improvement within our value-based care initiatives. You will lead clinical documentation and work across clinical, operational, coding, analytics, and payer teams to optimize documentation and risk capture.

Responsibilities include educating providers on best practices, guiding risk adjustment programs, and supporting population health metrics to improve quality outcomes

Qualifications

• Medical degree with relevant clinical expertise.
• Experience in risk adjustment, clinical documentation improvement, or related fields.
• Strong collaboration and communication skills across multidisciplinary teams.
