Job summary

Rise Community Hospital in Mansfield, TX is seeking a Rise Patient Advocate to support patients through the addiction recovery and behavioral health journey. This on-site role partners with the treatment team to assist admissions, coordinate benefits, and engage alumni and community resources.

The position offers daily and rotating shifts, full-time and PRN options, and a competitive hourly pay range. A compassionate, HIPAA-compliant communicator with strong de-escalation skills will thrive here.

Qualifications

• Education: High School Diploma or equivalent.
• Certifications: Current Basic Life Support (BLS) certification or ability to obtain upon hire.
• Experience: Direct working experience supporting individuals with behavioral health or substance use disorders.
• Understanding: Solid understanding of behavioral principles and recovery models.
• Communication: Professional telephone etiquette and clear written/verbal communication skills.
• Privacy: Strict dedication to patient privacy (HIPAA) and ethical healthcare compliance.

Responsibilities

• Build enduring patient relationships to support through the treatment process and conduct post-discharge follow-ups.
• Assist admissions by answering calls, pre-assessments, verifying benefits, and coordinating intake scheduling.
• Engage alumni and the recovery community with weekly meetings and events.
• Collaborate with the onsite clinical team to ensure resources and accurate records.

What You’ll Do

You will serve as a continuous champion for patients across the entire care continuum—balancing compassionate advocacy, admissions support, and recovery community engagement:

• Patient Support & Relationship Building: Build enduring relationships to support patients through the treatment process, communicate treatment plans with families, resolve patient concerns, and conduct post-discharge follow-ups to track long-term recovery.
• Admissions & Benefit Navigation: Answer incoming calls and emails, conduct phone pre-assessments, verify insurance benefits, guide potential clients past barriers to care, and coordinate intake scheduling.
• Alumni & Community Engagement: Facilitate weekly alumni recovery meetings, organize community alumni events, and maintain an active, supportive presence across the recovery network.
• Interdisciplinary Collaboration: Partner with the onsite clinical treatment team to ensure patients receive essential resources, maintain accurate client records, and assist with meal preparation and nourishment service as needed.

What We’re Looking For

• Education: High School Diploma or equivalent required.
• Certifications:
• Current Basic Life Support (BLS) certification (or ability to obtain upon hire).
• Experience: Direct working experience supporting individuals with behavioral health or substance use disorders.
