Job summary

White House Clinics is seeking a Triage Nurse to join a primary care team and facilitate communications between patients and care providers. You will relay messages, coordinate care plans, and manage lab results and instructions with care and accuracy.

You will educate patients about lab results, assist with pre-authorizations for medications, and triage medical concerns within your scope of license. A valid RN/LPN license and BLS are required.

Qualifications

  • Current Nursing License (RN or LPN) issued by the Kentucky Board of Nursing.
  • American Heart Association Basic Life Support (BLS) certification.
  • Experience in medical office triage or patient communication preferred.

Responsibilities

  • Act as intermediary between patients calling the health center and care team.
  • Relay patient messages to providers and coordinate plan of action.
  • Serve as primary contact for lab results and related instructions.
  • Educate patients on lab results and provider instructions within scope.
  • Assist with medication pre-authorizations and prescription requests.

Join us today!PRIMARY FUNCTIONTriage Nurses are member of a primary care team responsible for facilitating communications between patients calling into the office and the care team. The Triage Nurse will utilize both assessment and technical skills to ensure patient needs are met, questions answered, and care team communications are delivered. The Triage Nurse acts as a liaison on behalf of our patients to provide high quality healthcare.PRINCIPLE DUTIESAct as the intermediary between patients calling into the health center and the care team.Relay patient messages to providers, request plan of action for patient care, and relay direction from care team to patient.Serve as primary point of contact for patient lab results and other instruction from the care teams to patients.Provide education to patients regarding lab results, clarification of provider instruction (within scope of license), and answer general medical questions.Assist with pre-authorizations for medications and assist providers with prescription requests.Work within scope to triage patient medical concerns, schedule medical visits when appropriate, provide basic care and education to patients, and direct patients to higher level of care as needed.Maintain accurate and thorough patient records.Record vital statistics and other pertinent patient data as needed.Document every patient contact in electronic health record.Ensure all patient calls are answered, messages are returned, and all contact is documented within required timeframes.Assist Patient Access team members with in-person triage and assessment of walk-in patients. Work with providers to determine care plan for patient.Complete other duties within the scope of practices as assigned by the provider and/or supervisor.Provide superior customer service to all patients, external customers and partners, and co-workers. Serve as an ambassador of White House Clinics in all conversations and interactions with these parties.ACCOUNTABILITYReports to the Triage Manager, and ultimately to the Director of Clinical Support Services.SUPERVISIONNone.TYPICAL WORKING CONDITIONSMedical office environment with frequent contact with staff and the public. Exposed to blood, toxic or caustic chemicals. Exposure to communicable diseases, toxic substances, medicinal preparations, and other conditions common to a clinic environment. Position may involve dealing with angry or upset people. Work may be stressful at times.TRAVEL REQUIREMENTSRegular travel is not a requirement for this position. Employee may be asked to provide coverage at another clinic, on occasion.WORK HOURS37.5 hours per week, subject to wage and hour laws. Preference also given to candidates fluent in Spanish.Required License: Current Nursing License issued by the Kentucky Board of Nursing (RN or LPN); American Heart Association Basic Life Support Certification.QualificationsSuccessful applicants will be mature and well organized with pleasant personality and speaking voice. Knowledge of medical office procedures, basic computer skills, and patient flow in a primary care setting is preferred. Position requires skills in verbal and written communication, multi-tasking as well as proficiency in time management and organizational skills. Employee is responsible for maintaining requirements of certification and providing evidence of this to the organization as requested.
