Lake Butler Hospital is a critical access hospital in North Florida providing 24-hour emergency services, inpatient hospitalization, swing bed program, rehabilitation services, outpatient laboratory, and outpatient radiology (X-ray, ultrasound, and CT Scan) services to Union County and the surrounding counties. We are devoted to providing all members of our community with premier-quality health care in a compassionate and inviting environment.

We are seeking a clinician to join our team PRN at the Lake Butler Family and Pediatric Clinic. We provide primary care to children and adults care in a rural health care setting in collaboration with Lake Butler Hospital.

The position requires the following: Delivers primary medical care to a wide variety of patients in a Rural Health Clinic setting. The Clinician will be responsible for caring for patients, maintaining accurate and current patient records and scheduling and administering follow-up appointments to patients as required. The successful candidate will work as a team and will assist in delivering premium care to each and every patient. Performs initial patient screenings and patient history and physical examinations. Updates patient records and check records for accuracy at each patient appointment. Analyzes test data and determine the need for follow-up appointments and further treatment options. Delivers quality care while maintaining all company metrics pertaining to productivity. Serves as the resource patients need for ongoing care information, counseling and provide guidance for patients.

Requirements

  • Must have a valid Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) Florida license,
  • Must have Florida Medicare and Florida Medicaid numbers (or the ability to obtain).
  • Must have the ability to see ALL ages of patients (FAMILY PRACTICE)

Must have the following:

  • Ability to work within a team and take direction.
  • Must be familiar, or the ability to learn, the EHR medical charting system.
  • Strong written and interpersonal communication skills.

Position is PRN.
