Job summary

Hartford HealthCare at Home is seeking a Revisit Licensed Practical Nurse (LPN) to provide skilled nursing care to clients in home and community-based settings under the direction of a Registered Nurse. You will document progress in EMR and collaborate with primary nurses, physicians, and other disciplines to achieve optimal outcomes.

The role requires CT LPN licensure, a valid CT driver’s license with a good driving history, and a high school diploma or GED.

Qualifications

• Experience with Outlook, Word, and Excel preferred.

Responsibilities

• Provide intermittent skilled nursing services under the direction of an RN.

• Assist in evaluation of care by paraprofessional staff.
• Participate in agency committees and special projects.

As part of a person-centered care model, and under the direction of the RN, this role will provide skilled nursing care to clients in home and community-based settings ensuring the delivery of outcome based, high quality, cost-effective care. This role will collaborate with all team members to achieve exceptional outcomes.

Provide intermittent skilled nursing services in accordance with the established plan of care under the direction of an RN.

Provides support to patient/family utilizing teaching principles to promote and restore health and prevent disease and disability.

Records daily progress in EMR.

Reports to primary nurse, supervisor, physician, social worker, or therapist regarding cases.

Communicates/conferences With Primary Nurse And Other Disciplines.

Consults with physician regarding client needs and plan of care.

Assists in evaluation of care given by paraprofessional staff.

Participates on agency committees and special projects.

Qualifications

Minimum Requirements:

• High School diploma or GED
• Successful completion of a CT state approved LPN certificate program

Experience

• Computer skills including but not limited to Outlook, Microsoft Word, and Excel

Licensure

• Current and valid LPN license
• Valid CT driver’s license & a good driving history
