Job summary

Hartford HealthCare at Home in Connecticut is seeking a Visiting Licensed Practical Nurse (LPN) to join our homecare team in Watertown. This role emphasizes patient-centered care, collaboration with an interdisciplinary team, education, and documenting skilled services with advanced technology.

Qualifications

• Licensed Practical Nurse with valid CT LPN license.
• One year of LPN experience; homecare experience preferred.
• Valid driver’s license and reliable transportation required.
• Computer proficiency required.
