One Community Health is hiring a Retail Pharmacist to operate the 340B Retail Pharmacy within the Hood River clinic, on-site at The Dalles location. The role focuses on high-quality pharmaceutical care, safe medication use, and collaboration with a multidisciplinary team.

Responsibilities include dispensing, patient counseling, MTM, chronic disease management, immunizations, education, inventory, and regulatory compliance to optimize 340B benefits. Oregon licensure required; PharmD preferred.

Qualifications

• PharmD from an accredited college of pharmacy.
• Active and unrestricted Oregon Pharmacist License (or ability to obtain within 45 days).
• At least one year of experience in a retail pharmacy setting.
• Experience with the 340B Drug Pricing Program.

Responsibilities

• Dispense medications accurately with proper labeling and dosage instructions.
• Provide comprehensive patient counseling on medication use, risks, and adherence strategies.
• Conduct MTM and collaborate with providers to optimize therapy.

The Retail Pharmacist is a patient-focused professional responsible for operating the 340B Retail Pharmacy within the One Community Health clinic in Hood River, Oregon. This position will be full-time on site at our The Dalles Clinic location. This role provides high-quality pharmaceutical care, ensures the safe and effective use of medications, and collaborates with a multidisciplinary healthcare team.

Key Responsibilities

• Medication Dispensing:
  • Accurately dispense medications prescribed by clinic providers, ensuring proper labeling, packaging, and dosage instructions.
• Provide comprehensive counseling to patients on medication use, including potential side effects, interactions, and adherence strategies.
• Medication Therapy Management (MTM):
  • Conduct medication reviews, collaborate with patients and providers to optimize therapy, and address medication adherence issues.
• Clinical Collaboration:
  • Partner with healthcare providers and the clinical support team to deliver optimal pharmaceutical care, including offering therapeutic recommendations and drug information.
• Chronic Disease Management:
  • Support patients with chronic conditions (e.g., diabetes, hypertension, asthma) through medication management, follow-ups, and education on lifestyle modifications.
• Immunization Services:
  • Administer immunizations in accordance with state regulations and clinic protocols.
• Patient Education and Advocacy:
  • Educate patients on disease prevention, health promotion, and lifestyle changes to enhance medication effectiveness.
• Inventory Management:
  • Oversee medication and supply inventory, ensuring accurate stock levels, ordering, and storage in compliance with regulations.
• Regulatory Compliance and 340B Administration:
  • Ensure pharmacy operations comply with federal, state, and clinic standards.
  • Monitor pharmacy performance metrics and implement strategies to optimize 340B program benefits and expansion.
• Team and Organizational Support:
  • Uphold and promote One Community Health’s philosophy, mission, and core values.
  • Consistently follow and support established guidelines and procedures.

Minimum Qualifications

• Doctor of Pharmacy (PharmD) from an accredited college of pharmacy.
• Active and unrestricted Oregon Pharmacist License (or ability to obtain within 45 days of hire).
• At least one year of experience in a retail pharmacy setting.
• Prior experience with the 340B Drug Pricing Program.
• Comprehensive clinical knowledge and attention to detail.
• Strong passion for patient education and medication management.
• Proficiency in pharmacy management software systems.
• Excellent communication, problem-solving, and mentorship skills.
• Ability to work both independently and collaboratively within a diverse healthcare team.
• Commitment to confidentiality, professionalism, and workplace safety.

Preferred Qualifications

• Experience in chronic disease management and Medication Therapy Management (MTM).
• Previous experience in a community or migrant health center (FQHC) or similar healthcare setting.
• Proficiency with EPIC Willow Ambulatory or similar pharmacy management systems.
• Board of Pharmaceutical Specialties Certification (BCACP or BCPS)

Compensation

• Bilingual Differential: Additionally, we offer $0.75 per hour as compensation for bilingual proficiency in both English and Spanish.
