Responsibilities:
- Lead the care management for our residents by partnering and communicating with residents, families, physicians, healthcare providers, and the nursing team to provide the highest quality of care.
- Conduct hands-on assessments of residents on admission, quarterly, and as conditions change for ADL Restorative needs, and develop potential programs accordingly.
- Update Restorative Programs and goals as the resident's condition changes.
- Meet with CNAs monthly to discuss resident progress, new program development, changes in programs or goals, and the discontinuation of existing programs.
- Complete nursing portions of the MDS and other assessments such as skin, fall, and contracture risks on admission, quarterly, and during significant changes in resident condition.

Qualifications:
- Current and valid LPN license for the state of Illinois
- Experience preferred, but applications are encouraged from interested candidates
- Ability to communicate professionally both verbally and in writing
- Demonstrate a high standard of ethics
- Proficiency in computer skills and Microsoft Office
- Excellent customer service and communication skills
