Job description

We are currently seeking a skilled nurse to join our team as a Restorative Nurse. If you are looking for an exciting career opportunity where you’ll have the chance to make a difference in the lives of others, then we are the perfect fit for you!

Responsibilities:
  • Lead the care management for our residents by partnering and communicating with residents, families, physicians, healthcare providers, and the nursing team to provide the highest quality of care.
  • Conduct hands-on assessments of residents on admission, quarterly, and as conditions change for ADL Restorative needs, and develop potential programs accordingly.
  • Update Restorative Programs and goals as the resident's condition changes.
  • Meet with CNAs monthly to discuss resident progress, new program development, changes in programs or goals, and the discontinuation of existing programs.
  • Complete nursing portions of the MDS and other assessments such as skin, fall, and contracture risks on admission, quarterly, and during significant changes in resident condition.
