Elevate Care is seeking a compassionate Restorative Nurse (RN or LPN) to join our dedicated team in Des Plaines, IL. You will develop and implement personalized restorative care plans to help residents reach their highest level of independence.

Responsibilities include monitoring progress, providing therapeutic activities, educating residents and families, collaborating with the nursing and therapy teams, and diligently documenting progress. Equal opportunity employer.

Qualifications

• Active RN or LPN license.
• Experience in Restorative Nursing, Rehab, or Long-Term Care (Preferred).
• Strong assessment and critical thinking skills.

Responsibilities

• Develop & Implement Restorative Care Plans – Work with residents and the healthcare team to promote functional independence.
• Monitor & Evaluate Resident Progress – Track the effectiveness of restorative interventions and adjust care plans as needed.
• Provide Therapeutic Activities – Assist residents with exercises, mobility, and functional tasks to maintain or improve their abilities.
• Educate Residents & Families – Provide guidance on exercises, safety, and strategies to enhance recovery.
• Collaborate with Nursing & Therapy Teams – Ensure coordinated care across all disciplines.
• Document & Record Resident Progress – Maintain accurate and up-to-date records of care and progress.
