Job summary

Rivaya of Des Plaines is seeking a dynamic Restorative Nurse to coordinate and supervise restorative programs, ensuring compliance with facility policies and applicable regulations. This role is critical in maintaining residents' functional abilities and promoting person-centered restorative care.

You will develop restorative services, coordinate with therapists, and complete restorative assessments to improve mobility and safety for residents.

Qualifications

• Completion of a recognized Restorative Nursing Program or willingness to complete.
• Active Nursing License as required by the state.
• Current BCLS certification.

Responsibilities

• Develop restorative services with a person-centered care approach.
• Coordinate and supervise restorative programs to enhance residents’ mobility and function.
• Conduct restorative assessments to determine program eligibility and effectiveness.
• Complete MDS assessments related to restorative care.
• Create individualized restorative care plans for residents.
• Coordinate schedules and programs in collaboration with therapists.
• Educate and communicate restorative program goals to the nursing team.
• Oversee and schedule restorative interventions based on resident needs.
• Review fall assessments, reports, interventions, and care plans to enhance resident safety.
• Review bowel and bladder assessments and implement appropriate interventions and care planning.

Restorative Nurse Requirements:

• Completion of a recognized Restorative Nursing Program or willingness to complete
• Active Nursing License as required by the state
• Minimum two (2) years of nursing experience
• Current BCLS certification
• Strong organizational and leadership skills
• Ability to develop and oversee restorative programs to meet residents’ needs
